 Client information will be collected during your first visit to Lighting a New Way Counseling Services.  In order to save time, you are encouraged to download the following documents and bring the completed forms with you to your initial appointment. 

I am a provider for most insurance plans.   As a courtesy, claims will be submitted directly to your insurance company.  Please bring your insurance card with you to your first appointment. Your copay and any coinsurance are due on the date of service.  I accept payment by cash, check, or credit card.
